About Support at Home Services
On 1st November 2025, Support at home will replace the current home care packages programs. Support at home is a government funded initiative that supports individuals aged 65 and older in maintaining their independence at home.
The support at home program will feature a defined service list, consisting of 3 care support categories: Clinical supports, independence supports and everyday living supports.
The Support at Home program also introduces 3 additional support categories that attract their own separate funding amounts.

Clinical Support
Specialised services aimed at maintaining and regaining functional and cognitive capabilities. Services are delivered directly or supervised by accredited health professionals trained in evidence-based prevention, diagnosis and treatment to deliver quality care to older people.
Independence Support
Support to help older people manage daily activities that may become more challenging as they age. These services promote improved comfort and independent living. Services include transport, personal care, social support, and assistive technology.
Everyday Living Support
Support services that assist older people to keep their home in a liveable condition and enable them to stay connected with their local community while remaining independent in their own homes. Services include showering, allied health and transport to shops and medical appointments.
Additional Supports
Our aim is to assist you in staying at home as long as possible. Your Care Partner will advocate for you if it’s identified that you require additional services to keep you safe and comfortable at home. Under Support At Home, certain short-term services may be assigned to you, including Assistive Technology and Home Modifications (AT-HM), Restorative Care Pathway and End Of Life Pathway.
