This is about easing the load on families
We know that as a person nears end of life, care needs can increase quickly. People may lose mobility or continence, or require pressure care. This can be overwhelming for carers, especially when they’re already tired or trying to manage work and other responsibilities.
This program provides additional government-funded services to help families manage. It offers up to $25,000 in support over 12 weeks, including personal care, domestic assistance, respite, and more.
This is not about replacing palliative care
Benetas will work alongside existing palliative care teams, not instead of them. This support is designed to complement the care already in place, particularly where hands-on, practical help is needed at home. We’re not here to take over, we’re here to support.
What’s included
Services are flexible and will be tailored to meet the needs of each person and their family. Support may include:
- Personal care – bathing, toileting, pressure care
- Domestic assistance – cleaning, laundry.
- Nursing support – symptom monitoring, medication assistance
- Allied health – Physio or OT review of changing function and equipment
- Pastoral care – emotional and spiritual support for clients and carers
- Bereavement follow-up – check-ins with carers after death
Who is eligible?
This support is available to people who:
- Are receiving Support at Home programme services, and;
- Are likely to live three months or less (as determined by a medical or nursing practitioner), and;
- Have significant functional decline, measured as a Karnofsky score of 40 or below
Eligibility must be confirmed by a medical or nurse practitioner, who will complete the End-of-Life Pathway form.
How does it work?
- Up to $25,000 of funding is available per person
- Funding is available for up to 12 weeks, with a possible extension to 16 weeks if not fully used
- If the person is already receiving Support at Home services, their funding can be switched to this pathway following a review
This is one-time, time-limited funding. Once the episode ends, any unused funds do not carry over.
How to access End Of Life care funding
The fastest way is through your GP or specialist. Here’s what to do:
- Talk to your GP or nurse practitioner about eligibility
- Ask them to complete the End-of-Life Pathway form
- Submit it via:
- The My Aged Care provider portal, or
- Call 1800 200 422 to request a high-priority aged care assessment
