What Is Home Support Care Management?
Home Support Care Management includes the behind-the-scenes activities that make your aged care work smoothly and effectively.

Your dedicated Benetas Care Partner will:
- Create and update your personalised care plan
- Coordinate your home and community services and supports
- Monitor, review and evaluate how well your care is meeting your needs
- Advocate for you when changes or new services are needed
- Offer ongoing education and guidance
- Keep you and your family informed - every step of the way
It’s your care, tailored to your life - guided by someone who knows and understands you.
How It Works Under the Government Funding Model
Under the Support At Home program, Care Management is recognised as a core component of every funded package.
Here’s how it works:
- The Australian Government sets aside 10% of your total Support At Home funding to cover the cost of Care Management.
- This amount is automatically allocated by Services Australia and does not come out of your personal co-contribution.
- This ensures that every client receives dedicated support for planning, coordination and oversight — at no direct cost to you.*

Clinical Case Management
What is Clinical Cade Management and how is it different to Care Management?
Sometimes, your care needs may be more complex - for example, if you require ongoing nursing care, wound management, medication support, or multiple allied health services working together.
In these cases, we may provide Clinical Case Management - a specialised form of Care Management delivered by qualified Registered Nurses.
Clinical Case Management involves:
- Assessing your clinical and health needs
- Coordinating your nursing and allied health services
- Developing and updating clinical care plans
- Liaising with your GP, specialists, and hospital teams
- Monitoring your ongoing health outcomes and progress
*Unlike general Care Management, Clinical Case Management is charged at a higher professional rate and sits outside the 10% Care Management allocation. However, it is still funded from your Support At Home budget - meaning there’s no extra out-of-pocket cost unless your total service plan exceeds your available funds.
Clinical Case Management provides the extra layer of expertise and oversight needed when your health needs become more complex.
