The Restorative Care Pathway Under Support At Home
Starting 1 November 2025, the Restorative Care Pathway will replace the Short-Term Restorative Care (STRC). This new pathway is an additional funding separate from your regular support at home budget and needs to be accessed individually based on your needs. The Restorative Care Pathway offers up to 12 weeks of tailored support (up to 16 weeks for people in regional areas) to help older Australians regain strength, independence, and daily function. Eligible individuals can access this support twice in a 12-month period, but not in consecutive quarters.
What is Restorative Care?
Restorative care focuses on improving your strength, mobility, confidence and independence, so you can live well at home for longer.
Rather than just maintaining the status quo, it’s about looking forward - working together to support your recovery, your goals and your wellbeing.
With the right supports in place, many people find they can regain lost ability, reduce falls risk, and avoid unnecessary hospital visits
What Can Be Included in a Restorative Home Care Plan?
Allied Health & Therapy:
- Physiotherapy
- Occupational therapy
- Exercise physiology
- Podiatry
- Dietetics
- Speech therapy
Clinical and Nursing Support:
- Wound care
- Medication management
- Monitoring chronic conditions (e.g. diabetes, blood pressure)
- Post-hospital care
Personal & Social Support:
- Help with daily living tasks while you regain strength
- Social outings or group programs to reconnect and rebuild confidence
- Companionship to reduce isolation and support emotional wellbeing
Nutritional Support:
- Meal preparation assistance
- Healthy delivered meals to support healing and energy
Tailored to Your Goals:
Your restorative supports are based on your individual care goals. Whether that’s being able to:
- Walk confidently to the letterbox again
- Cook for yourself after surgery
- Rejoin your regular community group
- Feel stronger in your day-to-day life

Key Changes Under the New Restorative Care Pathway
Here’s what’s expected from November 1, 2025:
- Extended Support Duration
- More Government Funding
- Better Availability
Extended Support Duration:
Care plans will now span up to 12 weeks, with potential extensions to 16 weeks in specific circumstances, such as for individuals in regional or remote areas.
Increased Availability:
The number of available places will more than double, from 2,269 to 5,000 per quarter, enhancing access to restorative care services.
Comprehensive Funding :
Each participant will receive a budget of approximately $6,000 for the 12-week period, with the possibility of an additional $6,000 if further services are required. This funding covers allied health services, assistive technologies, and home modifications.
